Stockton Better Care Fund Update Additional documents: Minutes: Members considered a report that provided the Board with an update on the submission of the Quarter 2 report to the NHS England, on 31 October 2024. The Board also received a presentation providing case studies, detailing interventions and outcomes for Hospital Discharges, Support in the Community and Mental Health Support. Discussion and key points, included:- - Members agreed that keeping people at home, and out of hospital, was key. - Referrals to community services were increasing. - The Board agreed that additional support, provided to carers, was important, as any increased pressure on them, particularly elderly carers, could lead to a deterioration in their own health. RESOLVED that the submission and presentation be noted/endorsed. |

Members' Updates Minutes: There was a brief update on the Healthwatch review of dentistry. Further research at the Darlington Dentistry Centre had concluded and discussions were ongoing with the ICB. A regional report was likely to be available, early in the new year, and Stockton specific information would be shared with the Board. It was explained that the Primary Care Outreach Team: Hartlepool and Stockton Health had been successful in winning the Innovation and Improvement in Reducing Healthcare Inequalities, at the Bright Ideas in Health Awards. |
